2021-2022 CDDS REQUIRED POLICIES AND AGREEMENTS

Covid Safety/In-Studio Classes
You agree to the In-Studio Guidelines stated on website, including waiver, screening tool, mask requirement, arrival/exit process. These policies are strongly upheld at CDDS and are non-negotiable.Masks and distancing are required in-Studio for all classes- see website for exceptions.
Only staff and students permitted inside the Studio buildings.
In-Studio classes may transition to online classes at any time. Schedule will remain as close as possible. Tuition emails the same. No refunds.
You will notify the Studio immediately if your dancer is sick, exposed or tests positive for Covid-19 and will not attend class.
All government guidelines and additional safety precautions will be upheld.
Only students and staff permitted in Studio and use of bathroom.
You agree not to hold CDDS liable for injury or contraction of illness, including COVID-19.
A minimum number of students enrolled is required for class to take place. If a class does not meet enrollment requirements, an alternative day and time will be offered pending availability.
Make up classes TBD.

 Payments
Initial Enrollment: Your primary card on file will be charged Enrollment Fees as stated on website within 2-5 business days of registration receipt.
All payments made via Credit Card (Visa and Mastercard accepted only.)
There are no refunds. You agree there are no refunds regardless of absence or withdrawal.
Accounts must be up to date for student to participate in class or events.

Tuition
Monthly Tuition is due the 1st of each month via credit card payment. $20 late fee applies to past due accounts.
Monthly Tuition Auto Pay is preferred and automatically processes your primary card on file the 1st of each month.
Monthly Tuition is based on the 10-month September-June season and does not alter regardless of classes per month or absences.
Session Tuition will be charged to your primary card on file upon confirmation of class.
Tuition must be paid for student to participate in class or events.
No refunds.
June Recital costumes due by 12/1. See website for details.  

Online Classes
You release CDDS from all liability for any injuries taken place at home arising from or concurrent with my child or my participation in online classes.
Adult supervision in the same room is required for students under 7. Adult presence in the household is required for students 7 and older.
A minimum number of students enrolled is required for class to take place. If a class does not meet enrollment requirements, an alternative day and time will be offered pending availability.
Make up classes may be available in online classes.

Private Lessons
Payment Methods: Payment due upon confirmation of lesson. via credit card (Visa, Mastercard). No refunds.
Cancellation Policy: Notify Studio via email immediately if you need to cancel. Payment available as credit to account for future use if cancel at least 24hrs prior to lesson. No credit available if cancel within 24hrs of lesson. No refunds.
If an instructor is unavailable, a substitute will be provided and the lesson will still take place.

Delinquent Accounts
Accounts must be up to date for student to participate in class or events.
Accounts will be closed if delinquent for 30 days and CDDS will charge any card on file the owed past due balance.
Account must be settled to return to class.
If necessary, client agrees to pay all costs of collection, including attorney fees.
Contact info@carolyndutradancestudio.com with any financial concerns or requests. We are always here for our dance families and willing to work with you to come up with a fair and reasonable agreement.

Performances
There will be all-Studio Winter 2021 and June 2022 performances.
June Recital costumes due by 12/1. See website for details.

No Refunds
There are NO refunds on any payments made (Registration, Tuition, Costumes, etc..).
Attendance
Students and parents agree to hold good attendance as important.
Frequent absences may result in not being able to participate in a performance and repetition of level.
Call in advance if dancer will be absent.
Students may not arrive late or be dismissed early.
Makeup classes TBD. Students may make up a class for absences, not for days the Studio is closed (ie. Holidays)

Class Expectations
Students must display appropriate and respectful behavior at all times. "Disrupting the harmony" may result in dismissal.
Students will follow approved dress code.
A minimum number of students enrolled is required for class to take place. If a class does not meet enrollment requirements, an alternative day and time will be offered.

Liability/Medical/Safety
The safety and security of our dancers and families is of highest importance to us. The undersigned releases CDDS from all liability for any injuries or illnesses (including COVID-19) arising from or concurrent with my child or my participation in any classes or activities.
If a dancer is sick or has an injury preventing full participation in class they cannot attend.
A doctor's note is required for any absence due to injury and return to class after recovered.
Students with an injury require a doctors note to return to class.
Students in a cast/brace cannot participate in Acro class and require a doctors note to return.
Pending Miss Carolyn's approval, students in a cast/brace may participate in dance if a doctors note of approval is provided.
After cast/brace is removed, a doctors note approving participation in dance/acro is required to return to class.
If student is out due to injury/illness for extended period of time, they will be placed on Medical Leave and tuition may be paused. Student's spot may be forfeited in the class and if class is full upon return, will enroll in an available class.
CDDS is not responsible for lost or stolen articles.

Special Considerations
All children are welcome at the Carolyn Dutra Dance Studios.
Please provide us with the tools necessary to give your child the best possible experience at the studio by informing us of any and all special considerations - this may include information on allergies, accessibility concerns, behavioral, psychological or emotional conditions or other special needs. Details of any Individualized Education Plans (IEP) or Behavior Plans used with your child at school can often be helpful. Please put these notes under "Special Needs" in Student's Medical Information. Email to info@CarolynDutraDanceStudio.com upon enrollment.

Harmony Clause
Inappropriate behavior, language, disrespect to Studio members, staff or property, or disrupting the harmony of the Studio, is cause for dismissal whether by parent, child, or family member. There are no refunds.

Photo Release
Throughout the year, there may be times when the CDDS Staff, or the media, with the approval of the director, may take photographs or videos of students. These photographs or videos may appear in studio publications, on the website or on the studio Facebook page and Instagram. You can be assured that all photos are done in a professional and appropriate manner.I hereby grant CDDS permission to use my child's photograph and/or videotaped image for the purposes mentioned above.

Final Agreement
All CDDS members agree to uphold respectful and appropriate behavior at all times along with all Studio policies. Students, parents and guardians agree to follow all rules, regulations and policies of the Studio.
